I haven't put together a howto, yet. :)
I'll do that when I make another for my 5 x 8 index cards, using the full-size Moleskine Memo Pockets. It should be pretty easy to figure out how to make, though: cut out the memo pockets, cut a flap from a folder, paste in with strong glue.

It's just plain stupid and wasteful. He spent almost $20 on a perfectly fine notebook to gut it, rip it, and jam other stuff in it. It's not in the least bit clever, but overly complicated and ridiculous.
